Requirements for a Smart Postpaid Plan Application

You can directly apply for a Smart Postpaid and/or Smart Bro Postpaid account online. Simply go to the Smart Online Store and submit the following requirements:

1. A. Online Application Form

When applying for a plan online, just fill out the online application form instead of submitting SAF upon choosing preferred plan and phone/device.

B. Service Application Form (SAF)

If you wish to apply at any Smart Store, you can get a Service Application Form (SAF) should be completely filled-out and properly signed. The information provided must match the details of accompanying documents.

2. Proof of Identification (POI)
The POI can be any one of the following below. If you have a laminated Identification Card, ensure that it has your photo and signature:

a. Company ID
b. Government-issued ID

  • Unified Multi-Purpose ID (UMID)
  • SSS (Digitized)
  • Passport (with signature)
  • Driver's License
  • Firearm license
  • PRC ID
  • IBP ID
  • Digitized Postal ID

c. Exclusive Club Membership ID (Top Sports, Country Clubs)

3. Proof of Address (POA)
POA must not be more than three (3) months old at the time of application and can be any one (1) of the following documents:

a. Utility Bills (Electricity, Water, Cable, Telco, DSL)
b. Waybill or Proof of Shipping (Delivery Receipt)
c. Bank Statement of Account
d. Credit Card Bill / Statement of Account
e. Insurance Premium Bill / Statement of Account
f. Club Membership Bill

Alternatively, the following IDs with addresses may be accepted as POA:
a. Postal ID (Digitized)
b. Firearm License (Digitized)
c. Unified Multi-Purpose ID
d. Voter’s ID with Biometrics
e. PhilHealth ID
f. Top 8k Company ID (with home address)
g. Driver’s License (with Official Receipt)
h. Temporary Driver's License (with Official Receipt and Copy of expired license)

4. Financial Documents
Financial Documents must not be more than three (3) months old at time of application. Here is the list of accepted Financial Documents:

a. Certificate of Employment + 1 month payslip/s
b. Latest ITR (Form 2316) + 1 month payslip/s
c. Bank Certificate of Deposit with ADB for the last three (3) months
d. Bank Account Passbook, with last three (3) months transactions
e. Latest three (3) months Bank Statement of Account
f. Credit Card Statement of Account (SOA), issued within the last three (3) months
g. Latest 3 months Proof of Remittance + Proof of Relationship
h. Business documents ( Business permit, Latest ITR with Audited FS ) - for self-employed / business

Other documentary requirements may be requested based on the credit evaluation.
