You can visit any Smart Store to request for a Smart Postpaid SIM replacement. Bring one (1) valid ID and the SIM card requested for replacement. These requirements are also applicable in case you encounter a defective SIM card or if you wish to have an upgrade.
In case of lost or stolen SIM cards, please bring the following:
1 Valid ID
Duly Authorized Affidavit of Loss
Proof of Ownership (Eg. SIM Beds, PUK Slips or Personalized Smart Money Card)
Lost or damaged SIM card replacement and processing, within or beyond the 7-day warranty
is free of charge. Please check the SIM card activation date in reference to your 7-day warranty.
