Apply for personalized SMART Money Card


A personalized Smart Money card contains the cardholders name on it.

Subscribers can apply for a Smart Money card online, at any SMART Store or any Banco de Oro branch.

To request for a personalized SMART Money Card over-the-counter, please bring any of the following Proof of Identification at any Smart Store or BDO branch:

  • Drivers license
  • Passport
  • Major credit card with picture
  • Laminated company ID
  • Professional Regulation Commission (PRC) ID
  • Laminated SSS ID
  • Taxpayers Identification Number (TIN) card with picture
  • Un-expired school IDs (for applicants between 18 20 years old)
  • Postal ID

Kindly accomplish the SMART Money Application Form (SMAF) and pay a one-time processing fee of P100. The personalized card will be delivered to your preferred address after seven (7) days if within Metro Manila or fourteen (14) days if provincial.

Note: You may also opt to pick-up the personalized card at your preferred Smart Store, however the P100.00 processing fee still applies.

BSP REGULATIONS

  • Only a maximum of two Instant Card/Personalized Card accounts is allowed per individual as regulated by Banko Sentral ng Pilipinas (BSP).
  • Only a maximum of two (2) SMART Money accounts may be linked to a SMART mobile number.
